By submitting this form, I agree to the following:

1. I have uploaded a copy of my current Public Liability Insurance

2. I acknowledge that payment for my stall is non-refundable

3. I acknowledge that the placement of my stall location will be decided by the BAC Field of Quilts team, and that their decision will be final

4. I acknowledge that my stall fee payment includes one entry ticket to the event and that any person/s accompanying me will pay for their own entry ticket

5. I acknowledge that my stall fee payment includes one kitchen voucher per stall and that any person/s accompanying me will pay for their own kitchen voucher

6. I acknowledge that an email will be sent approximately one month prior to the 2024 BAC Field of Quilts event, outlining next stepson stall number allocated to me.

7. I acknowledge that photographs may be taken at this event, and that my image might be used in promotional material, and event report purposes. All photos of the event remain the property of the Boddington Arts Council, who may use them as and when needed
